Online Awards Entry Form
Instructions (please read)
- IMPORTANT: After filling out the form, you must PRINT the form BEFORE proceeding to PayPal to submit payment.
- Print three copies of this form. One copy should go with each of the books that go to the three judges in your entry’s category.
- You must fill out all required fields to print, pay the submission fee and submit the form.
- At the bottom of the form, you will be required to pay the $30/$35 submission fee. If you wish to pay by credit card, but DO NOT have a PayPal account, click “PayPal Checkout” and then select “Pay with Debit or Credit Card”. If you wish to pay using your PayPal account, click “PayPal Checkout” and login to your account.
- If multiple books are to be included in one total payment, please specify the author and title of each book in the Comments section of the Entry form.
- Printed proof of payment must be mailed to Gerry Plecki, SMA President, 33 King Henry Road SE, Poplar Grove, IL 61065.
- Awards Entry Form deadline for submission is Jan. 31, 2025.
Note: We need the author’s (not the editor’s) contact information to communicate with the winner. Submissions without the author’s contact information will be disqualified.
